Mestrado em Gestão dos Serviços de Saúde; O objectivo primário deste estudo é esclarecer a relação que existe entre a morbilidade e a qualidade de vida pós operatória, e como objectivo específico pretende caracterizar a morbilidade e a qualidade de vida dos doentes submetidos a cirurgia cardíaca. O estudo é prospectivo, em que 120 doentes submetidos a cirurgia cardíaca electiva foram acompanhados durante seis meses, pela aplicação do EuroQol-5D na véspera, 3 e 6 meses após a cirurgia. A tabela de pontuação de morbilidade pós-operatória foi aplicada no final do internamento. Dos doentes estudados 67,5% apresentaram pelo menos uma complicação após a cirurgia e 32,5% não apresentaram complicações, no entanto apenas 12,5% apresentaram uma ou mais complicações severas. A taxa de mortalidade operatória foi de 1,67% e o score de morbilidade de 1,69. Os resultados mostraram diferenças significativas na qualidade de vida nos três momentos estudados, revelando benefícios em todas as dimensões do EuroQol-5D, excepto nas “actividades habituais”. A relação entre a morbilidade e a qualidade de vida mostrou-se fraca, o que sugere fraca influência da morbilidade após a cirurgia na qualidade de vida futura. OBJECTIVE: To validate a quality of life scale, EuroQoL, on stroke patients. METHOD: 67 patients were scored simultaneously for EuroQoL-5 Dimensions (EQ-5D), NIH Stroke Scale (NIHSS) and modified Barthel Index (mBI). Pearson test was used to correlate each scale. Additionally, 31 patients were examined by two independent evaluators on the same day through application of EQ-5D. Kappa statistics were used to evaluate interobserver agreement. RESULTS: EQ-5D showed good correlation with both stroke severity (NIHSS, r= -0.404, P<0.001) and degree of impairment on activities of daily living (mBI, r=0.512, P<0.001). We noticed a good interobserver agreement (k>0.60) in all dimensions evaluated (P<0.01). CONCLUSION: We demonstrated that EQ-5D is reproducible and valid on evaluation of quality of life in patients post stroke in Brazil.

Background: Clinical studies employ the Unified Parkinson’s Disease Rating Scale (UPDRS) to measure the severity of Parkinson’s disease. Evaluations often fail to consider the health-related quality of life (HrQoL) or apply disease-specific instruments. Health-economic studies normally use estimates of utilities to calculate quality-adjusted life years. We aimed to develop an estimation algorithm for EuroQol- 5 dimensions (EQ-5D)-based utilities from the clinical UPDRS or disease-specific HrQoL data in the absence of original utilities estimates. Methods: Linear and fractional polynomial regression analyses were performed with data from a study of Parkinson’s disease patients (n=138) to predict the EQ-5D index values from UPDRS and Parkinson’s disease questionnaire eight dimensions (PDQ-8) data. German and European weights were used to calculate the EQ-5D index. The models were compared by R2, the root mean square error (RMS), the Bayesian information criterion, and Pregibon’s link test. Background: Major depressive disorder (MDD) is strongly associated with an impaired quality of life (QoL), which is itself affected by various factors. Symptom-oriented ratings poorly reflect the impact of disease on the QoL and level of functioning of the mental health of subjects. The purpose of this study was to assess health-related QoL (HRQoL) using preference-based measures in outpatients with MDD with regard to their remission achievement and clinical factors affecting the HRQoL. Methods: This was a cross-sectional observational study. We recruited 811 patients with MDD from 14 psychiatric outpatient clinics in Korea. They were divided into three groups as follows: a new visit group (n = 287), a remitted group (n = 235), and a non-remitted group (n = 289). The 17-item Hamilton Depression Rating Scale was used to assign patients to the remitted or non-remitted group. The general HRQoL was assessed with the EuroQol 5D (EQ-5D), using both the EQ-5D index score and the EuroQol Visual Analog Scale (EQ-VAS). Oral-specific measures are often preferred to examine outcomes of oral disorders. However, generic measures can add additional information, including health utility. The aim was to assess the Oral Health Impact Profile (OHIP), EuroQol (EQ-5D), and Assessment of Quality of Life (AQoL) instruments in relation to oral health in terms of their discriminative and convergent validities. Data were collected from adults, 30–61 yr of age, in Australia by mailed survey during 2009 and 2010, including the OHIP-14, the EQ-5D, and the AQoL, a range of self-reported oral health variables, and by self-rated oral and general health. Responses were collected from 1093 subjects (a response rate of 39.1%). The OHIP, the EQ-5D, and the AQoL were associated with oral health variables, with effect sizes ranging from 0.6 to 1.1 for the OHIP, from 0.3 to 0.5 for the EQ-5D, and from 0.4 to 0.6 for the AQoL. Introduction: The EQ-5D allows the achievement of two essential components of any measure of health-related quality of life to be used in cost-utility economic evaluations: (i) a profile describing the health status in terms of domains or dimensions; and (ii) a numeric value associated with the health status described.Aim: The Portuguese version of the EQ-5D questionnaire was completed in 1998, based on guidelines set by the EuroQol Group, including translation and back translation procedures. Despite its wide use in Portugal, until now it had not yet been published studies that initially led to the Portuguese version and the guarantee of acceptability, reliability and validity. Objectives: Using generic measures to examine outcomes of oral disorders can add additional information relating to health utility. However, different algorithms are available to generate health states. The aim was to assess UK-, US- and Australian-based algorithms for the EuroQol (EQ-5D) in relation to their discriminative and convergent validity. Methods: Data were collected from adults in Australia aged 30-61 years by mailed survey in 2009-10, including the EQ-5D and a range of self-reported oral health variables, and self-rated oral and general health. Results: Responses were collected from n=1,093 persons (response rate 39.1%). UK-based EQ-5D estimates were lower (0.85) than the USA and Australian estimates (0.91). EQ-5D was associated (p<0.01) with all seven oral health variables, with differences in utility scores ranging from 0.03 to 0.06 for the UK, from 0.04 to 0.07 for the USA, and from 0.05 to 0.08 for the Australian-based estimates. The effect sizes (ESs) of the associations with all seven oral health variables were similar for the UK (ES=0.26 to 0.49), USA (ES=0.31 to 0.48) and Australian-based (ES=0.31 to 0.46) estimates. Background: In the health sector, the EQ-5D has been widely used to evaluate health-related quality of life, mainly due to its simplicity, robustness and plasticity in calculating years of life adjusted for quality. Methods: However, its model has remained stable over time and knowledge of the influence of such biodemographical variables as age and gender has not been incorporated. This information, along with the well-known non-linear behavior of the VAS, which measures the self-concept of the 'General State of Health', forms the basis for a new model, which we present here from a sample of patients who were operated on for chronic low back pain. Results: These are obtained using multiple regression analysis are consistent in terms of goodness-of-fit given that this new model is able to explain 91.3% of the self-concept of the 'general state of health'. Conclusions: Above leads to an improvement of 21.6% compared to the available models more robust and meets most of the restrictions for this type of statistical modeling.

Las terapias no farmacológicas (TNF) dirigidas a la mejora del estado cognitivo y físico de los pacientes han sido objeto de numerosas investigaciones. Las intervenciones psicológicas con familiares, aplicadas de forma individual o en grupos de ayuda mutua, constituyen un recurso ampliamente presente en las asociaciones de familiares de enfermos de Alzheimer. BACKGROUND: The aims of the study were to compare the dimensions of oral-health-related quality-of-life measured by a generic health state measure, the EuroQol, and a specific oral health measure, the Oral Health Impact Profile. METHODS: Data were collected in 2001–02 from a random sample of South Australian dentists using mailed self-complete questionnaires. Dentists recorded the diagnosis of dental problems and provided patients with self-complete questionnaires to record the nature, severity and duration of symptoms using the EuroQol (EQ-5D+) and 14-item version of the Oral Health Impact Profile (OHIP-14) instruments. RESULTS: Data were available from 375 patients (response rate = 72%). The EuroQol items of mobility, self care and usual activities formed a separate cluster of variables, as did anxiety/depression and cognition, while pain clustered with items from the OHIP physical pain subscale. OHIP items tended to form clusters consistent with the subscales of social disability, physical disability, physical pain, functional limitation and psychological discomfort. The OHIP handicap items clustered between the OHIP social disability and physical disability subscales. The OHIP psychological disability items split between the social disability and psychological discomfort subscales. CONCLUSIONS: The observed clusters of variables empirically supported most of the conceptual dimensions of the OHIP. We aimed; 1) to determine the validity of the EuroQol 5D (EQ-5D) for the health-related quality of life (HRQOL) of Korean patients with type 2 diabetes, and 2) to identify associated factors of the HRQOL of these patients. Follow-up surveys were conducted for consecutive patients with type 2 diabetes. HRQOL was assessed using the EQ-5D and the Short Form-36 (SF-36). The validity of EQ-5D was assessed with the perspectives of known group, convergent and discriminant validity. Additionally, a linear mixed model using a backward elimination was used for identify associated factors. Of the 1,072 patients included in the first survey, 858 (80.0%) completed the questionnaires in the follow-up. In the known group validity, the problem rates in each EQ-5D dimension were highest among women, elderly people, and less-educated subjects. The Spearman's ρ between the EQ-5D and the SF-36 scales were larger in the comparable dimensions than those in the less comparable dimensions. In the final model, we found that sex, age, education, body mass index, atrial fibrillation, stroke, and retinopathy were statistically significant. Our data suggest that the EQ-5D is a valid tool for Korean patients with type 2 diabetes and that various factors could affect their HRQOL.

The objective of this study was to evaluate the quality of life (QOL) of patients with deep infiltrating endometriosis (DIE) using EuroQOL (EQ-5D) and its correlation with symptoms and locations of endometriotic lesions. One hundred and fifty-nine patients referred for DIE from January 2011 to April 2011 were prospectively invited to complete questionnaires evaluating symptoms associated with endometriosis as well as the EQ-5D questionnaire and health state. Patients also had locations of DIE evaluated by clinical examination and magnetic resonance imaging (MRI). All 159 patients completed the questionnaires. Item response rate was 91.1%. The most intense symptoms were dysmenorrhea (7.1/10), painful defecation (6.3/10), and fatigue (6.0/10). Mean (SD) scores were 77 (14) for the EQ-5D questionnaire and 63.4 (21) for the health state. A relation was observed between the EQ-5D questionnaire and the presence of dysmenorrhea, dyspareunia, cyclic pelvic pain, painful defecation, and diarrhea or constipation. Vaginal and rectal infiltrations were significantly associated with altered EQ-5D and health state scores. The EQ-5D questionnaire is easy to complete and well related to symptoms of DIE. Rectal and vaginal infiltrations were found to be determinant factors of altered QOL by the EQ-5D questionnaire and health state.

The study was aimed at evaluating the validity of impact measures among patients with Ménière’s disease (MD) with outcome variables of EuroQol generic health-related quality of life (HRQoL) measures (i.e., EQ-5D) by using Visual Analogue Scale (VAS) and EQ-5D index values. 183 members (out of 200 contacted) of the Finish Ménière Association returned the questionnaires that they had filled out. Various open-ended and structured questionnaires focusing on diagnostic aspects of symptoms and impairment caused by the disease were used. For activity limitation and participation restriction, standardized questionnaires were used. Open-ended questions on impact of the disease were asked, and subsequently classified based on the WHO-ICF classification. The general HRQoL was evaluated with EQ-5D index value and EQ VAS instruments. Correlation and linear regression analyses were used to explore the association between HRQoL and other aspects. Based on the explanatory power of different models the disease specific semeionic model provides the most accurate prediction in EQ-5D index calculations (38 % of the variance explained). La enfermedad de Parkinson (EP) es un trastorno neurológico, discapacitante y progresivo caracterizado por múltiples síntomas motores y no motores que contribuyen al deterioro en la calidad de vida del paciente. La diversidad de síntomas asociados a la enfermedad y su manejo afectan a la funcionalidad y las expectativas del individuo sobre su bienestar físico, social y mental, que son los componentes fundamentales de la calidad de vida relacionada con la salud (CVRS). El objetivo del presente trabajo fue identificar las principales dimensiones de la CVRS afectadas en una población con EP con un grado de afectación leve-moderado. Treinta y seis pacientes con EP completaron este estudio transversal. Se emplearon el estadio de Hoehn y Yarh, el Unified Parkinson's Disease Rate Scale, la escala de actividades de la vida diaria de Schwab y England y el Test Get Up & Go. La calidad de vida fue valorada con el EuroQoL-5D y con el cuestionario Parkinson's Disease Questionnaire-39 items. Las dimensiones del PDQ-39, excepto el dominio PDQ-39 Dolor, así como el cuestionario EuroQoL-5D se correlacionaron de manera significativa con la gravedad de la enfermedad. Objetivo del trabajo Analizar la percepción de los estados de salud de un grupo de la tercera edad del Hospital de Guápiles y construcción de tarifas de equivalencia temporal con base en dichos resultados. Materiales y métodos Estudio no experimental de corte transversal.Se aplicó EuroQol-5D a un grupo de 30 personas de la tercera edad del Hospital de Guápiles.Se utilizó un análisis descriptivo para caracterizar la población entrevistada y presentar los principales resultados de las variables de la Euroqol-5D. Introduction: chronic conditions modify perceived health in affected individuals. For this reason celiac disease, being a chronic condition, may impair health-related quality of life (HRQOL). Objective: to analyze the impact of celiac disease in affected individuals. Method: observational, cross-sectional, prospective study in patients with celiac disease by administering two HRQOL questionnaires: EuroQol-5D and GastroIntestinal Quality of Life (GIQLI). Results: 54 stable patients on a gluten-free diet for a median 60 months, and 9 newly diagnosed individuals still on their usual diet were included. Overall GIQLI score was significantly higher, meaning a better HRQOL, in treated celiac patients versus pre-treated celiac patients (3.1 [2.7-3.5] vs. 2.4 [2.1-2.6], p < 0.01). Similarly, EuroQol's health status preference value was also significantly better in treated patients (0.87[0.8-1.0] vs. 0.7 [0.5-0.8], p < 0.01). EuroQol's visual analogic scale had also better scores, representing a better perceived health, among treated patients (80.0 [70.0-90.0] vs. 65.0 [40.0-71.0], p < 0.05). In comparison to EuroQol-5D scores among the healthy Spanish population, values obtained for celiac patients under treatment are similar to those seen in the general population. ‣ Cambios en la calidad de vida tras UCI según grupo diagnóstico: Comparación de dos instrumentos de medida

Objetivo. Análisis de la calidad de vida relacionada con la salud (CVRS). Se evalúan los cambios en la CVRS previa al ingreso en la Unidad de Cuidados Intensivos (UCI) y a los 12 meses del alta según la categoría diagnóstica, así como la relación entre ambos instrumentos de medida. Diseño. Estudio prospectivo observacional. Ámbito. UCI polivalente de un Hospital Universitario de segundo nivel con 450 camas. Pacientes. Pacientes ingresados en la UCI más de 24 horas, durante un período de 18 meses. Variables de interés. Se recogen variables demográficas, diagnóstico al ingreso, nivel de gravedad (APACHE II), estancia, procedimientos, mortalidad y CVRS mediante los cuestionarios Short Form SF-36 (SF-36) y EuroQoL 5D (EQ-5D). El estado de salud previo al ingreso se evaluó de forma retrospectiva. Resultados. Un total de 189 pacientes responden ambos cuestionarios. La calidad de vida mostró un deterioro significativo a los 12 meses del alta de la UCI. Los pacientes con traumatismo craneoencefálico (TCE) y neurológicos se asociaron con peor CVRS al año del alta. Los pacientes con politrauma presentaron importantes limitaciones físicas y dolor, pero sin diferencias significativas en el aspecto emocional. La EQ escala visual analógica y el EQ tarifa mostraron en estos tres grupos una diferencia clínicamente relevante. Los pacientes con problemas respiratorios representaron el único grupo de pacientes que mejoró su CVRS. La comparación entre ambos instrumentos de medida demostró una fuerte correlación en la función física. En el aspecto emocional la correlación fue más débil. Conclusiones. La valoración de la CVRS en los pacientes ingresados en la UCI debe hacerse teniendo en cuenta la categoría diagnóstica. Ambos instrumentos (EQ-5D y SF-36) son capaces de detectar los cambios en la CVRS...

Objetivo: Describir el estado de salud autopercibido y la calidad de vida relacionada con la salud (CVRS) de individuos sanos en edad laboral, medir el cambio experimentado con el padecimiento de la gripe y valorarlo en términos monetarios. Método: Estudio observacional descriptivo mediante cuestionarios administrados a 50 individuos, en edad laboral, infectados por el virus de la gripe durante el año epidemiológico 2004-2005 y residentes en domicilios particulares de la ciudad de Logroño (España). Los pacientes completaron los cuestionarios en dos ocasiones: con gripe y una vez restablecidos. Se efectuó un análisis de las variables sociodemográficas, las dimensiones y los indicadores de CVRS del cuestionario European Quality of Life-5 Dimensions (EQ-5D). Los índices de utilidad de la CVRS se emplearon para el cálculo de los años de vida ajustados por calidad (AVAC) perdidos. Resultados: Por término medio, la reducción del índice de la CVRS debida al padecimiento de la gripe varió entre 0,37 y 0,65, en una escala que va del 0 (muerte) al 1 (salud perfecta). Una epidemia de gripe, sobre un grupo de 100.000 individuos, puede suponer una pérdida de 137 AVAC, que valorados en términos monetarios ascenderían a 2.722.609 euros. Conclusiones: "Actividades cotidianas" es la dimensión EQ-5D que más empeoró...
