OBJETIVO: avaliar características clínicas, patológicas e moleculares de carcinomas mamários em mulheres muito jovens em comparação a tumores de mulheres na pós-menopausa. MÉTODOS: foram selecionados 106 casos de câncer de mama de mulheres jovens e 130 casos de mulheres pós-menopausa. Foram analisados dados clínicos (idade ao diagnóstico, estadiamento, ocorrência de metástases, tempo de sobrevida global e livre de doença), anátomo-patológicos (tamanho do tumor, tipo e grau histológico do tumor primário) e marcadores moleculares (receptores de estrógeno e progesterona, HER2, p53, p63, citoqueratinas 5 e 14 e EGFR) com uso da imunoistoquímica empregando microarranjo de tecido. Foi analisada a relação entre as características clínico-patológicas, imunoistoquímicas e de sobrevidas global e livre de doença. RESULTADOS: as pacientes muito jovens apresentaram maior frequência de nuliparidade (p=0,03), maior diâmetro dos tumores (p<0,000), estadiamento clínico mais avançado (p=0,01), maior número de linfonodos positivos (p=0,001) e tumores pouco diferenciados (p=0,004). Objective: We evaluated the effects of soy isoflavone supplementation on hemostasis in healthy postmenopausal women. Methods: In this double-blinded, placebo-controlled study, 47 postmenopausal women 47-66 y of age received 40 mg of soy isoflavone (n = 25) or 40 mg of casein placebo (n = 22) once a day for 6 mo. Levels of factors VII and X. fibrinogen, thrombin-antithrombin complex, prothrombin fragments I plus 2, antithrombin, protein C, total and free protein S, plasminogen, plasminogen activator inhibitor-1, and D-dimers were measured at baseline and 6 mo. Background. The link between endogenous estrogen, coronary artery disease (CAD), and death in postmenopausal women is uncertain. We analyzed the association between death and blood levels of estrone in postmenopausal women with known coronary artery disease (CAD) or with a high-risk factor score for CAD. Methods. 251 postmenopausal women age 50-90 years not on estrogen therapy. Fasting blood for estrone and heart disease risk factors were collected at baseline. Women were grouped according to their estrone levels (<15 and >= 15 pg/mL). Fatal events were recorded after 5.8 perpendicular to 1.4 years of followup. Results. The Kaplan-Meier survival curve showed a significant trend (P = 0.039) of greater all-cause mortality in women with low estrone levels (< 15 pg/mL). Cox multivariate regression analysis model adjusted for body mass index, diabetes, dyslipidemia, family history, and estrone showed estrone (OR = 0.45; P = 0.038) as the only independent variable for all-cause mortality. Multivariate regression model adjusted for age, body mass index, hypertension, diabetes, dyslipidemia, family history, and estrone showed that only age (OR = 1.06; P = 0.017) was an independent predictor of all-cause mortality. Objective: Obesity is a major public health problem leading to, among other things, reduced functional capacity. Moreover, obesity-related declines in functional capacity may be compounded by the detrimental consequences of menopause. The aim of this study was to understand the potential effects of excess body mass on measures of functional capacity in postmenopausal women. Methods: Forty-five postmenopausal women aged 50 to 60 years were divided into two groups according to body mass index (BMI): obese (BMI, >= 30 kg/m(2); n = 19) and nonobese (BMI, 18.5-29.9 kg/m(2); n = 26). To determine clinical characteristics, body composition, bone mineral density, and maximal exercise testing was performed, and a 3-day dietary record was estimated. To assess quadriceps function, isokinetic exercise testing at 60 degrees per second (quadriceps strength) and at 300 degrees per second (quadriceps fatigue) was performed. Reduced aerobic fitness is associated with an increased risk of cardiovascular diseases among the older population. The aim of this study was to investigate the effects of LED irradiation (850 nm) applied during treadmill training on the maximal exercise tolerance in postmenopausal women. At the beginning of the study, 45 postmenopausal women were assigned randomly to three groups, and 30 women completed the entire 6 months of the study. The groups were: (1) the LED group (treadmill training associated with phototherapy, n = 10), (2) the exercise group (treadmill training, n = 10), and (3) the sedentary group (neither physical training nor phototherapy, n = 10). The training was performed for 45 min twice a week for 6 months at intensities between 85% and 90% maximal heart rate (HRmax). The irradiation parameters were 39 mW/cm2, 45 min and 108 J/cm2. The cardiovascular parameters were measured at baseline and after 6 months. As expected, no significant differences were found in the sedentary group (p ≥ 0.05). O envelhecimento normal é um processo contínuo, que desencadeia alterações psicossociais e biológicas, não afetadas por patologias. Estas mudanças podem desencadear perdas de ordem neuromuscular e óssea, influenciando o aparelho locomotor dos idosos. A osteoporose é uma doença que afeta sistêmica e progressivamente o esqueleto sendo as fraturas sua principal consequência, especialmente na coluna vertebral. A identificação destas fraturas pode ser realizada através da morfometria vertebral, analisadas através de raios-X (MRX) ou absorciometria de raios-X (MXA), que consideram morfologia da vértebra através da comparação da altura de seus contornos em 6 pontos diferentes. As mulheres, sobretudo na pós-menopausa, estão mais susceptíveis a fraturas. A prescrição de exercício, não deve somente agregar benefícios físicos, mas também deve considerar a predisposição ao surgimento de novos agravos, como as fraturas. Os exercícios de alta intensidade na água podem funcionar como um recurso seguro para uma população normalmente fragilizada pela idade. Genetic factors can also influence susceptibility to CVD. Mannose-binding lectin (MBL) is a component of the innate immune system and an activator of the complement cascade. We evaluated the association of genetic polymorphism of MBL (MBL2) on risk factors for CVD in postmenopausal women. Methods: In this cross-sectional study, 311 Brazilian women (age >= 45 years and amenorrhea >= 12 months) were included. Exclusion criteria: presence of previous or current CVD, insulin dependent diabetes, chronic kidney disease, autoimmune diseases and cancer. Clinical, anthropometric and biochemical assessments were performed to evaluate the cardiovascular risk factors. DNA was extracted from buccal cell and polymorphisms at codons 54 and 57 in the MBL2 were determined by polymerase chain reaction (PCR). O presente estudo analisou as alterações nos marcadores indiretos de dano muscular após uma sessão de ações excêntricas de flexores do cotovelo com diferentes velocidades de movimento, em mulheres jovens e na pós-menopausa. Participaram da pesquisa 37 mulheres, voluntárias, com idade compreendida entre 18 a 65 anos. Essas voluntárias foram divididas em quatro grupos. Grupo de mulheres jovens que executou ações excêntricas com velocidade rápida (n=10) e grupo de jovens que executou ações excêntricas com velocidade lenta (n=9). Grupo de mulheres na pós-menopausa que realizou as ações excêntricas com velocidade rápida (n=10) e grupo de mulheres na pós-menopausa que realizou as ações excêntricas com velocidade lenta (n=8). As ações excêntricas foram realizadas no braço não dominante, utilizando-se um dinamômetro isocinético, e consistiram em 30 ações excêntricas máximas divididas em cinco séries de seis repetições. A major problem associated with advanced adult age is the remarkable decline in functional capacity and the associated loss of independence (1). Muscle strength of the lower extremities is a major neuromuscular determinant of these losses and mobility (2). Vibration exercise is a new method of training with ameliorations on bone (3), balance (4), strength of lower extremities (5, 6) and neuromuscular performance (7), and it easily to apply on previously physically untrained and frail persons. We hypothesized that a low-frequency vibrating board for 8 months would be feasible and improve more the functional capacity and the muscle strength of lower limbs than a walking-based program in post-menopausal women.The present study shows that our WBV training program may have a muscular positive effect, improving explosive strength. Moreover, to our knowledge, the current study is the first had obtained these results using low-frequency vibration in postmenopausal women. WBV seems to require longer sessions to pursue relevant effects on isokinetic strength. Walking programs should be prescribed to achieve significant improvements in health status related with usual daily physical tasks.

BACKGROUND: Osteoporosis affects approximately 30% of postmenopausal women. Gastrectomy, pernicious anemia, and more recently Helicobacter pylori infection, have all been implicated in the pathogenesis of osteoporosis. A reduced parietal cell mass is a common feature in these conditions. AIM: To study a possible relationship between chronic gastritis, parietal cell density of the oxyntic mucosa and bone mineral density in postmenopausal women, as chronic gastritis, Helicobacter pylori infection and osteoporosis are frequently observed in the elderly. METHODS: Fifty postmenopausal women (61.7 ± 7 years) were submitted to gastroduodenal endoscopy and bone densitometry by dual energy X-ray absorptiometry. Glandular atrophy was evaluated objectively by the determination of parietal cell density. Helicobacter pylori infection was evaluated by histology, urease test and breath test with 13C. RESULTS: Thirty-two patients (64%) presented chronic multifocal gastritis, and 20 of them (40%) showed signs of gastric mucosa atrophy. Lumbar spine osteoporosis was found in 18 patients (36%). The parietal cell density in patients with and without osteoporosis was 948 ± 188 and 804 ± 203 cells/mm², respectively. Ten osteoporotic patients (55%) and 24 non-osteoporotic patients (75%) were infected by Helicobacter pylori. CONCLUSION: Postmenopausal women with osteoporosis presented a well-preserved parietal cell density in comparison with their counterparts without osteoporosis. Helicobacter pylori infection was not different between the two groups. We concluded that neither atrophic chronic gastritis nor Helicobacter pylori seem to be a reliable risk factor to osteoporosis in postmenopausal women.

SUMMARY Background: Age related declines in lower extremity strength have been associated with impaired mobility and changes in gait patterns, which increase the likelihood of falls. Since community dwelling adults encounter a wide range of locomotor challenges including uneven and obstmcted walking surfaces, we examined the effect of a strength 11 and balance exercise program on obstructed walking in postmenopausal women. Objectives: This study examined the effect of a weighted-vest strength and balance exercise program on adaptations of the stance leg during obstacle walking in postmenopausal women. Methods: Eighteen women aged 44-62 years who had not engaged in regular resistance training for the past year were recruited from the St. Catharines community to participate in this study. Eleven women volunteered for an aerobic (walking), strength, and balance training program 3 times per week for 12 weeks while 7 women volunteered as controls. We report sequential changes in bone mineral density (BMD) at the forearm, hip and spine in 340 consecutive postmenopausal women referred by 103 general practitioners and six specialists, and who were either untreated or being treated with calcium, estrogen, norethisterone or calcitriol for a median period of 25 months (range 11-52). The mean annual rate of change in BMD at the three sites was: 1.39% in 44 women on norethisterone; 0.94% in 107 women on estrogen (both p < 0.001); 0.24% (not significant) in 52 women on calcitriol; -0.53% in 92 women on calcium; and -1.06% in 45 women on no treatment (both p < 0.01). The mean annual rate of change at the three sites in the 295 treated women was 0.43%, which was significantly positive (p < 0.001) and was 1.49 percentage points more positive than in the untreated women (p < 0.001). The greatest mean difference between treated and untreated patients was seen at the forearm, where it was 2.16 percentage points (p < 0.001). This was significantly greater than the difference at the femoral neck (1.21 percentage points (p = 0.037)) and lumbar spine (1.10 percentage points (p = 0.044)). Smoking has been associated with low bone density, fractures and poor intestinal calcium absorption. Calcium absorption is a critical factor in calcium balance in postmenopausal women but the mechanisms causing decreased absorption efficiency in postmenopausal smokers are controversial and poorly defined. We performed a cross-sectional study of 405 postmenopausal women attending a clinic for the management of osteoporosis to compare intestinal calcium absorption efficiency, serum vitamin D metabolites and parathyroid hormone levels in postmenopausal women who had never smoked, who were smokers previously or who were current smokers, to examine the relationships between these variables in smokers. Two hundred and fifty-two of the women had never smoked, 79 had smoked previously and 74 were current smokers. The hourly fractional rate of calcium absorption was similar in non-smokers and those who had previously smoked. Radiocalcium absorption was less in the 74 smokers compared with the 331 non-smokers [0.60 (0.29 SD) vs 0.71 (0.27); p = 0.004], as were serum calcitriol (p50.001) and parathyroid hormone (PTH) (p50.01). There was no difference in the relationship between calcium absorption and serum calcitriol between smokers (r = 0.38) and non-smokers (r = 0.28); hence the impaired calcium absorption in the smokers was almost entirely attributable to suppression of the PTH– calcitriol endocrine axis. In postmenopausal women smoking is associated with a reduction in calcium absorption efficiency due to suppression of the PTH– calcitriol axis. ‣ Bone turnover markers in Spanish postmenopausal women: the Camargo cohort study

BACKGROUND. This cross-sectional study was performed to determine the reference ranges for two bone turnover markers -aminoterminal propeptide of type I collagen (P1NP) and C-terminal telopeptide of type I collagen ( - CrossLaps, -CTX)- in normal postmenopausal Spanish women as determined in serum by automated methods. METHODS. A community-based population of 1080 healthy postmenopausal women was evaluated. Data regarding risk factors for osteoporosis and fractures were collected by means of a structured questionnaire. Fasting serum levels of P1NP, -CTX, 25-Hydroxivitamin D (25OHD), and intact parathyroid hormone (iPTH) were measured on the Elecsys 2010 automated analyzer (Roche). BMD at lumbar spine, femoral neck and total hip was determined by DXA. RESULTS. The mean age of subjects was 63±9. Logarithmic transformation of both markers was performed to allow for normal distributions. Mid-95% ranges for P1NP and -CTX were 19-100 ng/ml and 0.112-1.018 ng/ml, respectively. Mean values of P1NP (47.7±19.9 ng/ml) were similar to those previously determined by the manufacturer of the assays, whereas -CTX mean values (0.387± 0.197 ng/ml) were lower. Both markers were higher among osteoporotic women. CONCLUSIONS. Values obtained from this well-characterized population study provide reference ranges for serum automated P1NP and -CTX in normal Spanish postmenopausal women.

OBJECTIVE: The aim of this study was to evaluate the effects of isoflavones on the skin of postmenopausal women. DESIGN: A prospective study was performed with 30 postmenopausal women before and immediately after the end of treatment with 100 mg/day of an isoflavones-rich, concentrated soy extract for six months. A skin punch was performed in the gluteal region for sample collection before and immediately after the treatment program. Morphometric determination of epidermal thickness, the papillary index (wrinkling), and the amount of dermal elastic and collagen fibers was assessed. In addition, the number of blood vessels in the sample was also evaluated. The paired Student's t-test was used for statistical analysis (P < 0.05). RESULTS: Isoflavone treatment resulted in a 9.46% increase in the thickness of the epidermis in 23 patients. In addition, the papillary index was reduced in 21 women. The papillary index was inversely proportional to skin wrinkling, i.e., there were a large number of papillae after treatment. The amount of collagen in the dermis was increased in 25 women (86.2%). In 22 women (75.8%) we observed that the number of elastic fibers increased. The number of dermal blood vessels was significantly increased in 21 women. Objective: This study aimed to analyse the food, energy and macronutrient intake of a group of postmenopausal women participating in a health-care-program.-- Methods: Subjects included were 38 healthy postmenopausal women aged between 46 and 60 years, recruited from the Menopause Program of the Madrid City Council. Physical activity, some anthropometric data and dietary information was obtained using a modified version of the dietary history method, which contained a 24-hour-recall and a food frequency questionnaire covering the preceding month as reference period. Dietary quality indexes, including those of the energy provided by macronutrients, alcohol and fatty acids and PUFA + MUFA/SFA and PUFA/SFA ratios were calculated.-- Results: This group consumed a diet very similar to the traditional Mediterranean diet. Intake of vegetables (415 ± 165 g/d), fruits (396 ± 178 g/d) and fish (131 ± 69 g/d) was high and a wide variety of these products was consumed. Potato and cereal (157 ± 76 g/d) intake was low. Dairy products, meat, poultry and eggs were only a modest part of this diet. The fat quality (PUFA + MUFA/SFA = 2.26) was satisfactory.-- Conclusion: The diet of this group of postmenopausal women attending a prevention program closely conforms to current nutritional guidelines.
