This paper reviews the historical development of public health policies in Brazil and the insertion of oral health in this context. Since 1988, Brazil established a Unified National Health System ("Sistema Único de Saúde" - SUS), which was conceived to assure access to health actions and services, including oral health. However, a history of lack of access to health services and the health problems faced by the Brazilian population make the process of building and consolidating the SUS extremely challenging. Since 2004, the Oral Health National Policy has proposed a reorientation of the health care model, supported by an adaptation of the working system of Oral Health teams so that they include actions of health promotion, protection and recovery. Human resources should be prepared to act in this system. The qualifying process must take in consideration knowledge evolution, changes in the work process and changes in demographical and epidemiological aspects, according to a perspective of maintaining a balance between technique and social relevance.

Introdução:O presente estudo toma como objeto as políticas públicas de saúde no campo das substâncias psicoativas ilícitas, em sua relação com os direitos humanos.A política predominante na área tem historicamente seguido os pressupostos da guerra às drogas, que infringe direitos humanos. Observa-se intensa discussão social sobre a questão e construção de paradigma contraposto, o da redução de danos. Pressupõe-se que essa problematização social vem ocasionando reflexos nas políticas públicas.Objetivo:analisar a política pública de saúde desenvolvida pelo Estado brasileiro no campo das substâncias psicoativas ilícitas, conforme se aproximem ou se afastem dos direitos humanos.Método: o estudo se inscreve no referencial do materialismo histórico dialético, que fundamenta o campo da Saúde Coletivade forma expressiva.Trata-se de um estudo de natureza qualitativa, cujo objetivo se insere na pesquisa explicativa-analítica, com a utilização do procedimento técnico da pesquisa documental, complementado com a opinião de três especialistas da área, através de entrevista com roteiro semiestruturado. Growing concern that public health laws may be inadequate to the challenges that confront public health practitioners has led to adoption of a Healthy People 2010 objective for improved laws and policies. It has also led to concerted efforts to strengthen the legal foundation for public health practice. We provide a historical study of the anti-alcohol public health poster in Poland between 1948 and 1990. Our case study illuminates public health policies under communism, with the state as the dominant force in health communication. Poland has a distinctive history of poster art, moving from a Stalinist phase of socialist realism to the diverse styles of the later Polish School. Quantitative and qualitative analysis of 213 posters establishes the major themes and differentiates community approaches, which depict the drinker as a social or political deviant, from those emphasizing individual risk. Medical issues were a minor theme, reflecting public policies geared more toward confinement than treatment. However, Polish School artists used metaphor and ambiguity, and references to the contested cultural symbolism of drink, to complicate and subvert the narrow propaganda intent. Thus, although apparently unsuccessful in restraining overall consumption, these posters offer valuable lessons for policymakers on the use of visual media in health campaigns.

Governors use State of the State speeches to publicly outline state status, progress, and policy priorities. The purpose of this study was to qualitatively analyze the public health content of State of the State addresses from 2009 to 2013. Speech transcripts were collected and uploaded into NVivo10. Speeches were coded using a priori, general thematic codes followed by detailed sub-coding of public health text. Data was organized by emergent thematic patterns. From 2009 to 2013, 48 governors presented 72 speeches with public health messages. Framing current public health problems and outlining strategies were two of the three main themes that emerged. Problems were often framed using data or emphasizing economic costs. Governors outlined prevention strategies on tobacco, nutrition, or physical activity through increased funding, policies, or partnerships. Speech content provides information on priorities and trends and can be used to inform advocacy efforts. Future research should link these priorities to outcomes of legislation.

The central government s policies, though well-intentioned, have inadvertently de-emphasized environmental health and other preventive public health services in India since the 1950s, when it was decided to amalgamate the medical and public health services and to focus public health services largely on single-issue programs. This paper discusses how successive policy decisions have diminished the Health Ministry s capacity for stewardship of the nation s public health. These decisions have introduced policies and fiscal incentives that have inadvertently enabled states to prioritize medical services and single-issue programs over broader public health services, and diminished the capacity of the public health workforce to deliver public health services. Diseases resulting from poor environmental health conditions continue to impose high costs even among the more affluent, and hinder development. There are many approaches to strengthening the public health system, and the authors suggest one that may require relatively little modification of existing structures and systems. The decision by Australia’s High Court to uphold the constitutionality of the country’s ground-breaking tobacco plain packaging laws has been heralded as a victory for national sovereignty over vested interests. However, the ability of governments worldwide to introduce and implement public health policies and laws is increasingly threatened by trade and investment treaties that privilege investors over governments and provide avenues for international corporations to challenge democratically enacted public health policies in different countries.

Objetivos: En España, en tiempos recientes, algunas voces han puesto en cuestión ciertas políticas de salud pública por posibles invasiones de la libertad personal. El objetivo del capítulo es responder a dichas voces con argumentos éticos y económicos que justifican tales políticas. Métodos: El presente trabajo plantea la corriente de opinión y sus características. Después, partiendo de Stuart Mill, expone los principios éticos de no maleficencia, beneficencia, autonomía personal y justicia, y algunos conceptos correlativos pertenecientes al plano de la eficiencia económica: efectos externos, monopolio, información incompleta y asimétrica, relación de agencia, bienes públicos y selección adversa. New York City approached the 1918 influenza epidemic by making use of its existing robust public health infrastructure. Health officials worked to prevent the spread of contagion by distancing healthy New Yorkers from those infected, increasing disease surveillance capacities, and mounting a large-scale health education campaign while regulating public spaces such as schools and theaters. Control measures, such as those used for spitting, were implemented through a spectrum of mandatory and voluntary measures. Most of New York City's public health responses to influenza were adapted from its previous campaigns against tuberculosis, suggesting that a city's existing public health infrastructure plays an important role in shaping its practices and policies during an epidemic.

Throughout the 20th century, US public health and immigration policies intersected with and informed one another in the country's response to Mexican immigration. Three historical episodes illustrate how perceived racial differences influenced disease diagnosis: a 1916 typhus outbreak, the midcentury Bracero Program, and medical deportations that are taking place today. Disease, or just the threat of it, marked Mexicans as foreign, just as much as phenotype, native language, accent, or clothing. A focus on race rendered other factors and structures, such as poor working conditions or structural inequalities in health care, invisible. This attitude had long-term effects on immigration policy, as well as on how Mexicans were received in the United States.

This study searches historical records for the formulation and implementation of public health policies in Brazil, focusing on those referent to family planning. Initial conclusions show that the policies were geared toward international controlling interests. Family planning today, though officially recognized as an inalienable right of every citizen, still reflects the contradictory political, economic and ideological interests of the ruling power.; O presente estudo faz um resgate histórico da formulação e implementação das políticas públicas de saúde no Brasil, com ênfase na política de planejamento familiar. Conclui que, esta, em seu início, atendeu a interesses controlistas internacionais e hoje, o planejamento familiar, embora oficialmente reconhecido como direito de cidadania, ainda reflete interesses contraditórios das instâncias políticas, econômicas e ideológicas de poder.

In this paper, there is a reflection on the tension between the principle of universality and the cultural specificity of users of the Public Health system, inserting the discussion about collective health in rural land settlements in this axis. After presenting some statistical data about health conditions in rural land settlements, ethnographic data on a project of university extension accomplished in a rural settlement located in the metropolitan area of Porto Alegre (RS) is presented. A dialogic model of intervention in the field of public health, respecting cultural differences and including alternative therapeutic practices to the biomedical model is discussed. Desde o início do processo de medicalização diversos esforços ocorreram para a perpetuação da força de trabalho. Uma das medidas para o exercício do controle estatal foi a institucionalização do parto. Com isso, o entendimento sobre os processos fisiológicos e patológicos referentes à gestante, ao parto e ao recém-nascido avançou nos campos científico e tecnológico, permitindo a intervenção médica para melhoria das condições de saúde materno-infantil. O presente estudo se propõe a identificar as principais legislações que fundamentaram a formulação e a execução de políticas públicas para a saúde materno-infantil no Brasil, a partir da década de 1980, além de estabelecer as relações dessas políticas com indicadores de mortalidade materna e neonatal, no período de 1996 a 2005. Uma pesquisa documental foi realizada para identificar as principais legislações aprovadas e políticas implementadas pelo Ministério da Saúde relacionadas à saúde materno-infantil; enquanto os dados referentes aos indicadores de saúde foram coletados nas bases de dados do SINASC e SIM e disponibilizados on-line. Análisis de políticas públicas puede ser entendido como análisis de las actividades realizadas por un gobierno con objetivo de intervenir en determinada realidad o problema social. La finalidad del estudio fue realizar revisión narrativa buscando identificar los conceptos y métodos utilizados para análisis de políticas. Fueron adoptados como descriptores: Políticas de Salud, Políticas Públicas de Salud, Formulación de Políticas y Análisis, investigando en las bases de datos: Literatura Internacional en Ciencias de la Salud, Literatura Latinoamericana y de Caribe en Ciencias de la Salud, Publisher Medline y Base de Datos de Enfermería. La investigación fue realizada en el periodo de junio de 2013 a marzo de 2014. Fueron elegibles 32 artículos para obtención de los datos, publicados entre 2008 y 2012. Después de criterios de exclusión, 05 artículos fueron analizados; de estos, resultaron tres categorías: Bases conceptuales; Bases metodológicas y Desafíos para la enfermería.
