Introdução: O Transtorno Obsessivo-Compulsivo (TOC) acomete cerca de 1% da população mundial, independente de sexo, raça ou religião. Representa uma preocupação de saúde pública significativa, pois comumente interfere no funcionamento social e vocacional, estando associado a um risco até quatro vezes maior de desemprego. O TOC é considerado um transtorno heterogêneo e tem sido enfatizada a necessidade de identificar subgrupos mais homogêneos de pacientes com o intuito de desvendar fatores etiológicos e estabelecer estratégias terapêuticas mais eficazes. Algumas características têm se mostrado promissoras na tentativa de determinar fenótipos mais homogêneos de pacientes com TOC. Entre elas, podemos citar a idade de início precoce dos sintomas obsessivo-compulsivos (SOC) e a presença de comorbidade com os transtornos de tiques (TT). Outra característica psicopatológica proposta para avaliar pacientes com TOC associado ou não a tiques é a presença de experiências subjetivas tais como incômodo, desconforto ou mal-estar que podem preceder ou acompanhar os comportamentos repetitivos, sejam eles tiques ou compulsões. A avaliação dessas experiências tem sido limitada pela diversidade de conceitos e pela ausência de um instrumento válido para a investigação dessas experiências subjetivas. Introdução: Educação é precisamente a atividade que pode expandir as estratégias de prevenção das Infecções Relacionadas à Assistência a Saúde (IRAS). As infecções da corrente sanguínea associadas a cateter (ICSC) são consideradas as mais importantes IRAS no cenário da assistência crítica por estarem associadas com altas taxas de mortalidade de pacientes sob cuidados intensivos. A interação existente entre entender como o conhecimento é adquirido e qual a melhor maneira de se ensinar, sugere uma mudança na prática tradicional do ensino. Recent studies have suggested that some patients with idiopathic dilated cardiomyopathy (IDC) are also afflicted with insidious forms of viral myocarditis. Participation of hepatitis C virus (HCV) in this process has been postulated. The objective of this study was to evaluate a possible association between hepatitis C virus and idiopathic dilated cardiomyopathy. Systematic review of the literature using electronic databases (MEDLINE, EMBASES, LILACS and COCHRANE) for the period from 1995 to 2005, limited to papers published in English, Spanish and Portuguese. Sixty-two papers were found, of which six were in accordance with the proposed methodology. After selection, the articles were classified by quality of data and number of variables studied. Most of the patients were male adults from 31 and 75 years old, who had ischemic cardiopathy excluded as etiology of the dilated cardiomyopathy. A significant association between dilated cardiomyopathy and hepatitis C virus was found in only two papers, both from Japan and by the same author. Most of the papers received low classifications, as they did not fulfill the systematization criteria.

In the future, there will be an increased number of cervical revision surgeries, including 4- and more-levels. But, there is a paucity of literature concerning the geometrical and clinical outcome in these challenging reconstructions. To contribute to current knowledge, we want to share our experience with 4- and 5-level anterior cervical fusions in 26 cases in sight of a critical review of literature. At index procedure, almost 50% of our patients had previous cervical surgeries performed. Besides failed prior surgeries, indications included degenerative multilevel instability and spondylotic myelopathy with cervical kyphosis. An average of 4.1 levels was instrumented and fused using constrained (26.9%) and non-constrained (73.1%) screw-plate systems. At all, four patients had 3-level corpectomies, and three had additional posterior stabilization and fusion. Mean age of patients at index procedure was 54 years with a mean follow-up intervall of 30.9 months. Preoperative lordosis C2-7 was 6.5° in average, which measured a mean of 15.6° at last follow-up. Infection in prosthesis surgery is very dangerous because it changes the patient’s prognosis. Differential diagnosis between septic and aseptic loosening is fundamental in order to apply the correct treatment. The correct diagnostic approach is still debated in literature. The aim of this study was to perform a review of literature evaluating positron emission tomography (PET) capacity to distinguish between septic and aseptic loosening in hip prosthesis. Research was done principally among medical archives. Five studies were selected which satisfied the required characteristics, and a weighted average of sensitivity and specificity of the different studies was determined. The fluorodeoxyglucose positron emission tomography (FDG-PET) sensitivity in individuating hip prosthesis infections was 82.8% and specificity was 87.3%. Positron emission tomography based on 2-fluoro-2-deoxy-D-glucose could be a valid option if research is able to find an uptake pattern specific for septic and aseptic loosening.

Our objective was to report an incidental finding of thyroid hemiagenesis in a patient who presented with a left neck mass, present a review of literature, and to discuss management of this diagnosis. Case report and review of literature. An internet-based literature search was performed via Pubmed with key words, “hemithyroid agenesis, thyroid hemiagenesis, absent thyroid gland”. Clinical, pathological, radiologic data and follow up information is reported. A 55 year old woman with a left neck mass presented to our service. A diagnostic CT scan of the head and neck revealed an unrelated finding of an absent left thyroid lobe. A review of the CT imaging did not reveal ectopic thyroid tissue. A complete physical examination including a flexible laryngoscopy was unremarkable. The neck mass was pathologically a granulomatous lesion on surgical pathology. Thyroid function tests were within normal limits. In follow-up, patient remains asymptomatic. Hemithyroid agenesis is most commonly associated with hyperthyroidism, although, hypothyroidism has been reported. Pathology that can be associated in the remnant thyroid lobe includes adenocarcinoma, adenoma, multinodular goiter, and chronic thyroiditis. In counseling patients, it is important to educate them regarding these associated conditions and offer appropriate work up if indicated. Incidental thyroid agenesis with a negative work up can then be observed.

Lichen planus is a chronic inflammatory mucocutaneous disease. Mucosal lesions are classified into six clinical forms and there is malignant potential for two forms of OLP; therefore, follow-up should be considered. There are many un-established etiological factors for OLP and some different treatment modalities are based on etiology. The aims of current OLP therapy are to eliminate mucosal erythema and ulceration, alleviate symptoms and reduce the risk of oral cancer. We have used review papers, case reports, cohort studies, and case-and-control studies published from 1985 to 2010 to prepare this review of literature.

Necrotizing cervical fasciitis is a rare, fulminating infection causing extensive necrosis of the subcutaneous tissue and fascial planes with resultant skin gangrene. To reduce the morbitity and mortality from this condition, it requires early recognition and aggressive surgical debridment with appropriate antibiotic therapy. The origin is generally odontogenic and presents more often in immunocompromised patients. This article presents a case with typical clinical features and appropriate management of this condition. An review of literature was carried out for microbiology, pathogenesis, clinical features, diagnosis, management and prognosis of this condition.

Lipodystrophy is a clinical disorder characterized by maldistribution of body fat. Hyperinsulinemia, insulin resistance, and abnormalities of glucose homeostasis are commonly described among these patients. Hyperinsulinemia is also involved in the pathogenesis of polycystic ovarian syndrome, a condition, described rarely in patients with lipodystrophy. Here, we describe 2 females of partial lipodystrophy who presented with features of polycystic ovarian disease. Both had severe hyperinsulinemia and irregular periods, one had hyperandrogenism and hirsuitism while the other was non-hirsuite.

Introduction. Population growth and increasing urbanization in earthquake-prone areas suggest that earthquake impacts on human populations will increase in the coming decades. Recent large earthquakes affecting large populations in Japan, Haiti, Chile and New Zealand are evidence of this trend and also illustrate significant variations in outcomes such damage and mortality levels. The objectives of this review were to describe the impact of earthquakes on human populations in terms of mortality, injury and displacement and, to the extent possible, identify risk factors associated with these outcomes. This is one of five reviews on the human impact of natural disasters. Methods. Data on the impact of earthquakes were compiled using two methods, a historical review from 1980 to mid 2009 of earthquake events from multiple databases and a systematic literature review of publications, ending in October 2012. Analysis included descriptive statistics and bivariate tests for associations between earthquake mortality and characteristics using STATA 11. Findings. The differential diagnosis for precocious puberty in a young female includes peripheral causes. This case report documents a rare cause of isosexual precocious puberty, a juvenile granulosa cell tumor of the ovary–and a brief literature review. A 7-year-old girl presented with rapid onset of pubertal development and elevated estradiol levels. Abdominal ultrasound revealed a mass in the right adnexa. Other causes of precocious puberty were excluded. Elective surgery was planned, but the patient presented to the emergency room with torsion of ovary. She underwent an exploratory laparotomy for tumor resection and right salpingo oophorectomy. Pathology reported a juvenile granulosa cell tumor of the ovary. Postoperatively, she experienced a cessation of vaginal bleeding and estradiol levels normalized. Early stage disease has good prognosis. Adjuvant chemotherapy is not indicated in this setting.

With an overall mortality approaching nearly 100 %, pancreatic cancer is still a challenging disease. A major proportion of pancreatic cancers are still diagnosed at the locally advanced or metastatic stage. Tumors originating from tail region present very late due to the absence of specific symptoms and signs. The usual sites of metastases are the liver, peritoneum and lungs, with occasional reports of metastases in other sites. There are very few documented cases of gastrointestinal cancers manifesting as metastases to muscle. This is a report on a case of pancreatic cancer presenting as metastases to skeletal muscle and multiple other viscera and review of literature for pancreatic cancer metastases.

Mayer-Rokitansky-Kuster-Hauser (MRKH) is a characteristic syndrome in which the Mullerian structures are absent or rudimentary. It is also associated with anomalies of the genitourinary and skeletal systems. Its association with gonadal dysgenesis is extremely rare and appears to be fortuitous, independent of chromosomal anomalies. We report such a case in a 21-year-old girl who presented primary amenorrhea and impuberism. The endocrine study revealed hypergonadotrophic hypogonadism. The karyotype was normal, 46, XX. No chromosome Y was detected at the fluorescence in situ hybridization (FISH) analysis. Internal genitalia could not be identified on the pelvic ultrasound and pelvic magnetic resonance imaging. Laparoscopy disclosed concomitant ovarian dysgenesis and MRKH syndrome. There were no other associated malformations. Hormonal substitution therapy with oral conjugated estrogens was begun. The patient has been under regular follow-up for the last two years and is doing well.

Esophageal lymphoma is a rare condition, accounting for less than 1% of all gastrointestinal lymphomas. Primary extra nodal esophageal lymphoma constitutes less than 0.2% cases of the total esophageal lymphomas. The definition of primary GI lymphoma has differed among authors. The etiology of the disease is unknown, with the role of Epstein-Barr virus being controversial. The common symptoms of patients with esophageal lymphoma include dysphasia, odynophagia, weight loss, chest pain or present as a result of complications. Burkitt’s lymphoma is one of the fastest growing human malignancies, with a 100% replication rate. Endemic, sporadic (non-endemic) and immunodeficient variants have been recognized. The diagnosis of Burkitt’s lymphoma relies on morphologic findings, immunophenotyping results, and cytogenetic features. Burkitt’s lymphoma is usually treated with LMB-96 protocol depending on the risk stratification. We present a case of primary esophageal Burkitt’s lymphoma, which has been successfully treated with LMB-96 protocol. An extensive review of literature did not reveal a single case of esophageal Burkitt’s lymphoma. To the best of our knowledge this is the first case report in the world literature with diagnosis of primary esophageal Burkitt’s lymphoma.

The early computer-aided design/computer-aided manufacturing (CAD/CAM) systems were relied exclusively on subtractive methods. In recent years, additive methods by employing rapid prototyping (RP) have progressed rapidly in various fields of dentistry as they have the potential to overcome known drawbacks of subtractive techniques such as fit problems. RP techniques have been exploited to build complex 3D models in medicine since the 1990s. RP has recently proposed successful applications in various dental fields, such as fabrication of implant surgical guides, frameworks for fixed and removable partial dentures, wax patterns for the dental prosthesis, zirconia prosthesis and molds for metal castings, and maxillofacial prosthesis and finally, complete dentures. This paper aimed to offer a comprehensive literature review of various RP methods, particularly in dentistry, that are expected to bring many improvements to the field. A search was made through MEDLINE database and Google scholar search engine. Objective: People who have limited access to dental care may present to non-dental health practitioners for dental treatment and advice. This review synthesised the available evidence regarding the use of non-dental health practitioners for oral health problems and the services provided by non-dental health practitioners to manage such presentations. Methods: PubMed and CINAHL databases were searched using key search terms to identify all relevant quantitative and qualitative English-language studies published between 1990 and March 2014. Snowballing techniques were then applied whereby the reference lists of retrieved articles were searched for other relevant citations. Grey literature was searched via Google using the same search terms to identify unpublished work and government reports. Results: Of the 43 papers which met the review criteria, 25 papers reported on the use of non-dental health practitioners for oral health problems and 18 on dental care education and training for non-dental health practitioners. Four reports were located from the grey literature on the involvement of non-dental health practitioners in the management of oral health care. A rare case report of a plexiform unicystic ameloblastoma in a 3-year-old girl stimulated us to conduct a review of literature to understand the correlation of this tumor with various factors such as that of age, sex, histopathological correlation, and its incidence rates pertaining to children and adolescent population. This is a case report of ameloblastoma in a 3-year-old patient, along with a literature review of ameloblastoma in relation to age. A computerized literature search using Medline was conducted for published articles on treatment of ameloblastoma. MeSH phrases used in search were ameloblastoma AND age; ameloblastoma AND children. The search was restricted to published articles from 1970 to 2010, as the histological features were not clearly defined until 1st edition of WHO histological classification of odontogenic tumors of 1971, search parameter was also set to select literatures under English language only. We report a 17-year-old girl evaluated for primary amenorrhea. Cytogenetic analysis of the peripheral blood lymphocytes revealed normal autosomes with 46X inv (Y) confirming the diagnosis of Turner's syndrome with Y cell line. Treatment was initiated with conjugated estrogen while recommending bilateral prophylactic oophorectomy to the patient. One year later the patient presented with abdominal mass, biopsy of the specimen following resection confirmed dysgerminoma originating from right ovary with no invasion or metastasis. The literature is reviewed with regard to the various pathogenetic mechanisms proposed for the development of germ cell tumors in ovary, the cytogenetic findings and recommendations to handle such scenario.

The UP's constitute an important problem of health, particularly in ICUs. The nursing is loaded from the start of "Modern Nursing" responsibility or "blame" for the development of PU in hospitalized clients. In last years, this responsibility has been divided between the other members of the health team to be identified to the problem multicausality. The study aimed to examine articles, published in national and international scientific journals, about the use of risk-assessment scales for pressure ulcer on admission of customer as one of the nursing care instruments. This is a systematic review of literature with a qualitative approach, which used the electronic databases MEDLINE, LILACS, BDENF, SCIELO, OBJN and the Nursing School Library in Aurora Afonso Costa, using the time limitation period 2003-2008. In the study 11 items were selected and analyzed. It was concluded that the scales have been widely used as tools for risk assessment to predict the risk to develop PU. Based on the results of the analyzed studies, the use of the Braden scale has increased reliability (sensitivity and specificity) in predicting risk for development of UP.
