INTRODUÇÃO: A lesão intraepitelial cervical de alto grau causada pelo HPV, precursora do câncer cervical, é facilmente diagnosticada e seu tratamento pode ser realizado de maneira ambulatorial, sem muitas complicações. Mesmo assim, o seguimento das pacientes tratadas deve ser feito de maneira criteriosa e sistemática para que a recorrência ou persistência da doença não passe despercebida. Conseguir identificar o grupo de pacientes com maior probabilidade de recorrência ou persistência da doença facilitaria sobremaneira esse seguimento, diminuindo o ônus econômico e psicológico que o seguimento generalizado produz. O objetivo deste trabalho é identificar marcadores que possam indicar o grupo de pacientes com maior possibilidade de recorrência da lesão intraepitelial cervical de alto grau. O infarto agudo do miocárdio (IAM) é definido como uma necrose do miocárdio resultante de um comprometimento agudo de sua irrigação sang?ínea. As manifestações de insuficiência cardíaca (ICC) são comuns em pacientes com doença arterial coronariana (DAC) aguda ou crônica, acarretando significativa morbidade e mortalidade. O objetivo foi avaliar a função global e regional do ventrículo esquerdo (VE), através da Ressonância Magnética (IRM) com a técnica dos marcadores miocárdicos na evolução clínica dos pacientes na fase tardia do infarto da parede anterior do VE. Foi realizado seguimento longitudinal prospectivo da evolução da função da parede do ventrículo esquerdo. Foram avaliadas e comparadas entre si as várias etapas evolutivas de pacientes encaminhados pela Unidade Clínica de Coronariopatia Crônica e Unidade de Cirurgia Torácica e Cardiovascular do Incor-FMUSP. Entre dezembro de 2000 e fevereiro de 2003, estudamos 24 pacientes (19 homens e cinco mulheres), idade média de 54,33 ± 10,11 anos. Although several international guidelines recommend exercise to reduce symptoms of Major Depressive Disorder (MDD), data concerning the long-term effects of exercise in depression are still extremely scarce. Therefore, and in order to assess these long-term effects, we have evaluated treatment-resistant MDD patients previously enrolled in a moderate intensity 12 week exercise program at 3 and 6 months after the end of the program, regarding HAMD17, BDI, GAF, CGI-S, WHOQOL-Bref and SF-36. Results show that 47% of patients in the exercise group continued to exercise at follow-up. Those who continued to exercise maintained the same depression and functional parameters they showed after the 12 week exercise program, which were all improved compared to the initial values (p < 0.05). Those who stopped exercising showed worse HAMD17, GAF and CGI-S (p < 0.05) at 6 months follow-up than at the end of the exercise program. All patient groups maintained their QoL scores at 6 month follow-up compared to the scores at the end of the 12 week exercise program, regardless of continuing to exercise or not. Therefore, we suggest that positive effects of exercise on depression and functioning of treatment-resistant MDD only persist if exercise is continued over time. CONTEXT AND OBJECTIVE: The concept that the presence of atypical squamous cells cannot exclude high-grade squamous intraepithelial lesions (ASC-H) was introduced in the 2001 Bethesda System of cervical cytology classification. This nomenclature defines cervical cancer precursor lesions. The objective of this study was to investigate the colpocytological-histological results from a three-year follow-up conducted on a cohort of women with reports of ASC-H who were attended during 2005-2006 at clinics of the Southern Metropolitan Healthcare Service of Santiago, Chile. DESIGN AND SETTING: Prospective cohort study at primary healthcare clinics in Santiago, Chile. METHODS : Colpocytological-histological follow-up was conducted over a three-year period on 92 women with cytological reports of ASC-H who were attended at primary healthcare clinics during 2005-2006. This trial examined the optimal setting for follow-up of patients after treatment for colon cancer by either general practitioners or surgeons. In all, 203 consenting patients who had undergone potentially curative treatment for colon cancer were randomised to follow-up by general practitioners or surgeons. Follow-up guidance recommended three monthly clinical review and annual faecal occult blood tests (FOBT) and were identical in both study arms. Primary outcome measures (measured at baseline, 12 and 24 months were (1) quality of life, SF-12; physical and mental component scores, (2) anxiety and depression: Hospital Anxiety and Depression Scale and (3) patient satisfaction: Patient Visit-Specific Questionnaire. Hypothesis: Laparoscopic 90° anterior partial fundoplication for gastroesophageal reflux disease achieves equivalent results to laparoscopic Nissen fundoplication. Design: A multicenter, prospective, double-blind randomized clinical trial with a minimum of 5 years’ followup. Setting: Nine university teaching hospitals in 6 major cities throughout Australia and New Zealand. Participants: One hundred twelve patients undergoing primary antireflux surgery were randomized to undergo either laparoscopic Nissen fundoplication (52 patients) or anterior 90° partial fundoplication (60 patients). Interventions: Laparoscopic Nissen fundoplication with division of the short gastric vessels or laparoscopic anterior 90° partial fundoplication. Main Outcome Measures: Blinded assessment at 1 and 5 years’ follow-up of clinical outcome for postoperative heartburn, dysphagia, gas-related symptoms, and satisfaction with the surgical outcome. Analog scales ranging from 0 to 10 were used to assess symptom severity. Results: Ninety-seven patients underwent follow-up at 5 years. Three others died during follow-up, 4 refused follow-up, and 8 were lost to follow-up; 89% remained at 5-years’ follow-up. At 5 years’ follow-up, mean analog scores for heartburn were 2.2 for anterior fundoplication vs 0.9 for Nissen fundoplication (P=.003). There were no significant differences between the groups for dysphagia scores. BACKGROUND  Although the hope is that many perinatal interventions are performed with an ultimate aim to improve the long-term health and development of the child, long-term outcome is rarely used as a primary end-point in perinatal randomised controlled trials (RCTs). OBJECTIVE  To evaluate how often and with which tools long-term follow-up is performed after large obstetric RCTs. SEARCH STRATEGY  We searched the Cochrane Library for Cochrane reviews published by the Cochrane Pregnancy and Childbirth Group for reviews on interventions that aimed to improve neonatal outcome. SELECTION CRITERIA  Reviews on perinatal interventions that were not performed to improve the condition of the neonate were excluded. We limited our review to RCTs with more than 350 participating women. For each included study, we checked in Web of Science as to whether the researchers had reported on follow-up in subsequent publications. DATA COLLECTION AND ANALYSIS  Relevant information was extracted from these RCTs by two reviewers using a predefined data collection sheet. All information was analysed using SPSS 17.0 (SPSS Inc., Chicago, IL, USA). Background: Early diagnosis and improved treatment outcomes have increased breast cancer survival rates that, in turn, have led to increased numbers of women undergoing follow-up after completion of primary treatment. The current workload growth is unsustainable for breast cancer specialists who also provide care for women newly diagnosed or with a recurrence. Appropriate and acceptable follow-up care is important; yet, currently we know little about patient preferences. The aim of this study was to explore the preferences of Australian breast cancer survivors for alternative modes of delivery of follow-up services. Methods: A self-administered questionnaire (online or paper) was developed. Die vorliegende Studie verfolgt den 10-Jahres-Verlauf von 215 alkoholabhängigen Patienten nach einer Entwöhnungstherapie in der Universitätsklinik für Psychiatrie und Psychotherapie Tübingen. 10 Jahre nach der Indexbehandlung waren 66% der Gesamtstichprobe am Leben, 13% waren verstorben, über 21% konnte zum Untersuchungszeitpunkt keine Information gewonnen werden. Bei 59% ist ein Rückfall bekannt, 23% waren dauerhaft abstinent, bei 18% ist das Trinkverhalten unbekannt. 43% der Gesamtstichprobe (N=93) konnte ausführlich befragt werden. Diese unterschieden sich bei der Aufnahme kaum von den nicht befragten. Zu 13,5% konnte keinerlei Information gewonnen werden. Im Verlauf hatte sich die Situation der 93 befragten Patienten insgesamt verbessert: 43% waren dauerhaft abstinent, und im Jahr vor der Befragung hatten 63% keinen Alkohol konsumiert. Da viele Patienten nur sehr kurze Rückfälle hatten, wurden die Abstinenzzeiten kumuliert. Über 75% der Patienten hatten weniger als 1 Monat Trinkzeit. Bezüglich der Mortalität hatten vor allem alkoholbezogene Parameter sowie die Depressivität eine prognostische Bedeutung. Für die Rückfälligkeit waren neben alkoholbezogenen Merkmalen insbesondere die Prognoseeinschätzung des Arztes und Aspekte der Therapiemotivation sowie die psychische Beeinträchtigung aussagekräftig. Os estudos de follow-up a consumidores de drogas permitem conhecer fatores como alterações nos padrões de consumo, a efetividade de intervenções, entre outros. Contudo, um dos principais problemas subjacentes a esta técnica é conseguir taxas de resposta satisfatórias. Com base numa meta-análise bibliográfica o presente artigo pretende: 1) descrever uma amostra de 220 artigos internacionais de follow-up a consumidores de drogas; 2) testar preditores para a taxa de resposta nesta amostra. Conclui-se que existe uma relação significativa entre a taxa de resposta e a existência de intervalos regulares entre as vagas do follow-up, e que a existência de intervenção, isto é, a tentativa de alterar comportamentos dos participantes, não tem efeito na taxa de resposta.; Follow-up studies applied to drug users allow us to know factors such as changes in consumption patterns, the effectiveness of interventions, among others. Summary Objective: to examine the association between preoperative body weight, adherence to postsurgical nutritional follow-up, length of postoperative period, and weight loss during the first 18 months among adults who have undergone bariatric surgery. Methods: a retrospective cohort study was conducted on 241 consecutive patients who underwent open Roux-en-Y gastric bypass (RYGBP) from January 2006 to December 2008, in a teaching hospital in São Paulo (Brazil). Data were collected through hospital records review and the variables analyzed included sex, age, immediate preoperative weight, adherence to postsurgical nutritional visits and length of postoperative period. Proportional body weight reductions during the 18-month follow-up period were examined using generalized estimating equations. Results: 81% (n=195) of participants were female, with overall mean age of 44.4 ± 11.6 years, mean preoperative weight of 123.1± 21.2 kg and mean preoperative body mass index of 47.2± 6.2 kg/m2. The overall adherence to postoperative follow- up schedule was 51% (95%CI: 44.5-57.5%). Preoperative body weight and adherence were not associated with proportional weight reduction (Wald’s test p > 0.18). Weight loss leveled off at the end of the 18-month follow-up period for both compliant and non-compliant patients (Wald’s test p = 0.00). A cirurgia de ambulatório procura adicionar humanização às mais valias da cirurgia convencional, permitindo a admissão e a alta no mesmo dia, bem como o regresso precoce das pessoas ao seu ambiente familiar habitual. O seguimento ou follow-up é realizado pelo enfermeiro, por intermédio de um telefonema entre as vinte e quatro e as quarenta e oito horas após a alta, assegurando os cuidados pós-operatórios. AIMS: Neonatal intensive care is expensive, and thus it is essential that its long-term outcomes are measured. The costs of follow-up studies for high-risk children who survive are unknown. This study aims to determine current costs for the assessment of health and development of children followed up in our research programme. METHODS: Costs were determined for children involved in the research follow-up programme at the Royal Women's Hospital, Melbourne, over the 6-month period between 1st January 2012 and 30th June 2012. The time required for health professionals involved in assessments in early and later childhood was estimated, and converted into dollar costs. Costs for equipment and data management were added. Estimated costs were compared with actual costs of running the research follow-up programme. RESULTS: A total of 134 children were assessed over the 6-month period. The estimated average cost per child assessed was $1184, much higher than was expected. The estimated cost to assess a toddler was $1149, whereas for an 11-year-old it was $1443, the difference attributable to the longer psychological and paediatric assessments. The actual average cost per child assessed was $1623. The shortfall of $439 between the actual and estimated average costs per child arose chiefly because of the need to pay staff even when participants were late or failed to attend. CONCLUSIONS: The average costs of assessing children at each age for research studies are much higher than expected. BACKGROUND: The Magpie Trial compared magnesium sulphate with placebo for women with pre-eclampsia. 10,141 women were recruited, 8804 before delivery. Overall, 9024 children were included in the analysis of outcome at discharge from hospital. Magnesium sulphate more than halved the risk of eclampsia, and probably reduced the risk of maternal death. There did not appear to be any substantive harmful effects on the baby, in the short term. It is now important to assess whether these benefits persist, and to provide adequate reassurance about longer term safety. The main objective of the Magpie Trial Follow Up Study is to assess whether in utero exposure to magnesium sulphate has a clinically important effect on the child's chance of surviving without major neurosensory disability. Other objectives are to assess long term outcome for the mother, and to develop and assess appropriate strategies for following up large numbers of children in perinatal trials. Mothers and their children attended the Women's and Children's Hospital for anthropomorphic examination. Information on lifestyle factors was collected by questionnaire. Descriptive statistics examined differences between respondents and non-respondents and multiple regression models examined associations with blood pressure. Results: Blood pressure and body measurements were available on 45 and 43% of eligible women and children, respectively; questionnaire data were available on 50% of this cohort. At follow up, there were no differences between treatment groups in blood pressure in the women or their children. Introdução: As técnicas mais comumente empregadas em análise de sobrevida que utilizam dados censurados são o estimador produto limite de Kaplan-Meier (KM) e o modelo de riscos proporcionais de Cox. Estas técnicas têm como suposição que a causa da perda de seguimento seja independente do tempo de sobrevida. Objetivo: O presente estudo visa a analisar o efeito das perdas de seguimento nestas duas técnicas. Material e Métodos: O estudo foi realizado utilizando-se o banco de dados dos pacientes cadastrados no Registro Hospitalar do Hospital do Câncer de São Paulo em 1994. Foram elaborados 28 bancos de dados simulando perdas informativas e não informativas. A perda informativa foi simulada transformando os óbitos em vivos, na proporção de 5 a 50%. A perda não informativa foi simulada através do sorteio de 5 a 50% do total do banco. O estimador de Kaplan-Meier (KM) foi utilizado para estimar a sobrevida acumulada no primeiro, terceiro e quinto ano de seguimento, e o modelo de riscos proporcionais de Cox para estimar as hazard ratio (HR). Mestrado em Auditoria; O trabalho do auditor interno culmina com a elaboração do seu relatório. No entanto, é importante efetuar a monitorização da implementação das recomendações, de preferência através da realização do follow-up. Este trabalho adicional nem sempre é efetuado, tanto por falta de recursos, como por falta de tempo, sendo dada prioridade à elaboração de novos trabalhos de auditoria. Pelos motivos enunciados, seria interessante apurar a verdadeira utilização do follow-up pelos auditores internos, bem como aferir as opiniões que têm sobre a sua aplicação. Neste trabalho foi efetuada uma revisão da literatura aplicada ao tema escolhido, dando um retrato atual sobre o trabalho de auditoria interna, sobre os seus relatórios e sobre o follow-up. Posteriormente foi elaborado um inquérito que permitisse obter as informações necessárias para ir ao encontro dos objetivos definidos. Foram recolhidas 42 respostas de auditores internos que desempenham funções em Portugal. Foi possível concluir que os inquiridos atribuem grande importância ao follow-up e que a maioria o utiliza para efetuar o acompanhamento de grande parte dos relatórios emitidos. Objective: This study compared the hospital and follow-up costs of patients who have undergone endovascular (EVAR) or open (OR) elective abdominal aortic aneurysm repair. Methods: The records of 195 patients (EVAR, n = 55; OR, n = 140) who underwent elective aortic aneurysm repair between 1995 and 2004 were reviewed. Primary costing data were analyzed for 54 EVAR and 135 OR patients. Hospital costs were divided into preoperative, operative, and postoperative costs. Follow-up costs for EVAR patients were recorded, with a median follow-up time of 12 months. Results: Mean preoperative costs were slightly higher in the EVAR group (AU $961/US $733 vs AU $869/US $663; not significant). Operative costs were significantly higher in the EVAR group (AU $16,124/US $12,297 vs AU $6077/US $4635; P < .001); this was entirely due to the increased cost of the endograft (AU $10,181/US $7,765 for EVAR vs AU $476/US $363 for OR). Postoperative costs were significantly reduced in the EVAR group (AU $4719/US $3599 vs AU $11,491/US $8,764; P < .001). Background and Objectives: Obsessive compulsive disorder (OCD) is a frequent psychiatric disorder. Despite its significant influence on personal development, little is known about its long-term course in children and adolescents. The aim was to follow children and adolescents with OCD for 7 years and to compare patients gaining remission with patients experiencing symptoms. Methods: The study was a prospective follow-up study in which the patients were interviewed 7 years after their initial contact with the healthcare system. The patients had been described at baseline. Results: Among the 95 participants, 53 participants described symptoms at follow-up and 44 participants described remission. More patients with persistent symptoms described an anxious personality trait. The schizoid/compulsive personality traits were represented at a higher rate in the group with persistent symptoms than in the group with remission symptoms. Important predictive factors included predisposition to OCD, the occurrence of comorbid disorders and older referral age. The occurrence of magic obsessions and repetitive compulsions increased the risk of OC symptoms at follow-up. The group “late responders” differed from the non-responder group by predisposition to depression.
